This week I am talking with Jacob Reider, MD (@JacobR), CEO of Alliance For Better Health (@Alliance_PPS), a New York Delivery System Reform Incentive Payment (DSRIP) care transformation initiative. Dr. Reider is a family physician who wants the world to be more healthy – a man after my own heart! He previously served as the Deputy National Coordinator for Health Information Technology at the US Department of Health and Human Services.
The Alliance focuses on collaborative care in the community to create health equity replacing our fragmented system that continues to work as designed which as Jacob puts it needs to fill the physical facilities that we build to satisfy the economics. They are moving the needle away from Fee for Service (FFS) to Value-Based Care (VBC) and working to turn the hospital to be more like a fire station, who’s services are only accessed when all the other earlier and cheaper interventions have failed. They have invested heavily in Social Determinants Factors as the core contribution for 80% of health challenges
His incremental learning points include
- Early learning of what does not work (your first idea should be a sacrificial lamb)
- Creating a curated list of participants in any program
- Building trust
