This week I am talking with Maria Panagioti, Ph.D., (@MariaPanagiot83) Senior Lecturer at The University of Manchester who works in the Greater Manchester Patient Safety Translational Research Center under the Division of Population Health, Health Services Research & Primary Care. Her paper BMJ: Prevalence, severity, and nature of preventable patient harm across medical care settings: systematic review and meta-analysis, (good news this is open access) was published in the BMJ in July 2019 caught my attention especially in the context of the error meme (LINK MY BLOG) discussion I had with Chadhi Nabhan (LINK)

We dig into the concept of “Preventing non-preventable risk”. If you struggle with this phrase as an oxymoron like I did you need to listen in to hear why it’s not and the importance of this in healthcare going forward? We discuss the overall quantification of harm, how to categorize it and the gaps currently in our understanding of the harm that miss an international perspective and focuses mostly on hospital-based missing Primary care and Psychiatry to include Psychological harm
